BACKACHE (CERVICAL ,LUMBAR SPONYLITIS ,SLIP DISC ,SCIATICA )
 

Back-ache is the most common of all the musculo-skeletal complaints. It is estimated that at least 50% of people in Western industrialized countries will suffer from back pain at some time of their life. Acupuncture, gives excellent result . These symptoms constitute a group of ailments which cause significant morbidity and have different manifestations depending on area involved.

In neck as cervical spondylosis- patient complaint of pain,stiffness, pain radiating in arms ranging from headache, dizziness, tinnitus. Spondylitis of thoracic spine -leads to pain in the chest, cardiac, irregularities, abdominal pain, hyperacidity.

Spondylitis of lumbar spine- causes lower backache, sciatica, pain in the hips, thighs, knees, the calves and the ankles which are often misdiagnosed as a whole range of other ailments.

Slip disc in neck or lower back is becoming very common and is a serious problem in computer professionals and in modern medicine surgery is advised after conservative treatment has failed which may give short term relief , recurrences are common and many post surgery complications eg loss of urine control may occur , so by acupuncture surgery can be avoided if patient in early stages . A proper diagnosis must be made, otherwise the patient is treated for other ailments without any success because the underlying problem has not been diagnosed and treated.

 
CAUSES
  • Excessive or Inadequate physical work or exercise -    computer professionals.
  • Posture–wrong posture (prolonged sitting/standing).
  • Trauma–road accident ,fall from height.
  • Spondylitis, slip disc, canal stenosis, Lumbar muscular strain.
  • Repeated Pregnancy and childbirth.
  • Exposure to-cold/rainy season–insufficient clothes as    fashion or after swimming,
   Emotional stress.
  • Excessive sexual activity.
  • Obesity-very common.
  • Urinary and pelvic infection.
  • Varicose veins.
  • Deficiency– know a days it is becoming very common among youth due to staying long
   time in dark. Calcium, vitamin-D leads to osteoporsis [ common in menopause
   period ]
  • Ankylosing spondylitis.
  Lower back-ache can be treated perfectly adequately and successfully according to the Chinese diagnosis without any reference to western medicine especially where cause is unknown.
   
SCIATICA
  Is pain radiating down the lower limb, pain is along the posterior aspect of the leg (U.B. Channel) in 40% cases. Along the lateral aspect of the leg (G.B. Channel) in 40% while in 10% it is along the antero-lateral aspect (St. Channel) while in the rest of the patients. It may be along a different line altogether. Some times the pain is partly on the pathway of U.B. Channel and partly on the pathway if G.B. Channel. In cases of disc lesion and I have astonishingly got 80% good results. This made us review the concept of mechanical problem causing sciatica. Results of acupuncture are excellent even in severe PIVD and lumbar canal stenosis and surgery could be avoided in majority cases. Due to these findings I have been made to think that the cause of sciatica should be obstruction in the flow of energy in the yang channels of lower limb particularly U.B. & G.B. Channels. PIVD or Lumbar canal stenosis could be a precipitating or aggravating factor. Since then now I have been treating all cases of lumber canal stenosis, disc lesion or lumbago sciatica syndrome much more confidently with extremely good results.  
   
TREATMENT
  • Acupuncture combined with Acuyoga gives better & quicker results-Asanas
   (Pawanmuktasana, Supta Vajrasana Tadasana kati-chakrasana, makarasana), all
   backward bending asana are useful, forward bending asanas to be done carefully to
   limited extent, Pranayama (breathing exercise)is very essential as it helps to increase
   level of.
  • If there is underlying osteoporsis, the pain is not be completely relieved unless calcium
   is taken in diet-milk or milk products or calcium tab and sun bath for vitamin D.
  • Body weight control–is essential if obesity is contributing cause, acupuncture reduce
   weight by rectifing hormonal balance ,decreasing appetite ,remove water retention , so
   weight reduction on a lasting basis .
   
PROGNOSIS AND PREVENTION
  Acupuncture can be extremely effective in the treatment of both acute and chronic back-ache. It sometime produces extraordinary results even when there are severe structural imbalances in the spine .The duration of the complaint seems to be less relevant in back-ache than is other diseases many cases of very chronic back-ache of, say, over 20 year’s duration are times cleared in a few sessions. Acute attacks from sprain or invasion of Damp-Cold can be cleared in a few treatment sometimes even only one. If, however, the acute attack is a recurrence of a chronic problem, the treatment will take much longer: usually about 10-15 sessions. When the back-ache is accompanied by sciatica it will usually take longer to treat, if there is associated muscular weakness of legs it has to be combined specific acuyoga exercises to give good results . Mostly cases of chronic backache of long duration without obvious cause respond very well to acupuncture.
   
ARTHRITIS
  • OSTEOARTHRITIS -It is a sign of aging process but now a days it is seen in younger age due to sedentary life style. Obesity ,Diabetes , Thyriod disorders may predispose to early arthritis. It commonly affects knee , spine ,shoulder .
  OSTEOARTHRITIS - KNEE JOINT   This is the commonest disorder in the aged person (40 – 45 years). Knee being weight bearing joint, obese people are definite sufferer. Patient gets pain which aggravates on walking or long-standing. It is also interesting finding that if a patient starts walking after rest, he gets more pain during first few steps but the joint movement is more easy and pain lessens a bit in further steps. Quadriceps wasting may occur , knee joint may get swollen due to effusion. Besides acupuncture treatment , knee exercises are important to tone up muscles. Early acupuncture treatment can prevent knee replacement surgery.
FROZEN SHOULDER- There is limitation of movement of shoulder it is more common in diabetic and hypothyroid patients, results with acupuncture treatment combined with shoulder exercises are excellent .
   
RHEUMATOID ARTHRITIS
  It is a chronic inflammatory, destructive and deforming polyarthritis associated with systemic disturbance, and the presence of circulating antiglobulin antibodies , rheumatoid factors .It is common in the 3rd and 4th decades but and no age group is exempted. The prevalence in developed countries is about 3% with a female to male ratio of 3:1. It is common in ladies after child birth especially if delivery takes place during winter & rainy weather, it affects intially finger joints, later on other joints with swelling & pain if not treated in time it may result into deformity & limitation of movement, family history may be positive There is significant increase in HLA-DR4 in patients and their families associated with an approximately fivefold increase in relative risk in persons with these genetic markers. In the majority of patients the onset is insidious with joint pain, stiffness and symmetrical swelling of a number of peripheral joint with exacerbations and remissions, atypical, asymmetrical and incomplete forms are not uncommon . Initially pain may be experienced only on movement of joints, but rest pain and especially early morning stiffness are characteristic features of all kinds of active inflammatory arthritis. As the disease advances, pain, muscle spasm and joint destruction result in limitation of joint motion, joint instability and deformities. In intial stage the deformities are correctable. Rheumatoid arthritis is a systemic disease- loss of appetite, weight loss, lethargy and myalgia occur commonly throughout its course and may precede the onset of articular symptoms by weeks or months.Osteoporosis, muscle weakness and wasting occur adjacent to inflamed joints and more diffusely as part of the systemic disturbance. According to TCM Bi syndrome ( Rheumatoid arthritis ) is due invasion of wind ,cold ,humidity factors depending on extreme climatic conditions and lowered body resistance, so the basic principle is to expel pathogenic factor and tonify liver and kidney vital energy and results are very good and curative in early stages before deformities are formed . Modern medicine only gives symptomatic relief.
   
ANKYLOSING SPONDYLITIS
  In its typical form this is a chronic inflammatory arthritis with a predilenction for the sacroiliac joints and spine and characterized by progressive stiffening and fusion of the axial skeleton ( Bomboo-stick like spine ). It is a disease with a peak onset in the second and third decades and a male to female ratio of about 4:1. More than 90% of affected persons carry the histocompatibility antigen HLA-B27. There is a greatly increased incidence in the 1st degree relatives of patients with ankylosing spondylitis, psoriatic arthritis, inflammatory bowel disease and Reiter’s syndrome.

Clinical features. The onset is usually insidious with recurring episodes of low back pain and stiffness sometimes radiating to the buttocks or thighs. Characteristically the symptoms are worse in the early morning and following inactivity. Occasionally the onset may be acute, resembling a lumbar disc protrusion. A few patients present with symptoms referable to the dorsal or cervical spine but such cases usually reveal evidence of previous sacroiliitis and lumbar spine involvement.

Early signs include failure to obliterate the lumbar lordosis on forward flexion, pain on sacroiliac compression, and restriction of movements of the lumbar spine in all directions. As the disease progresses, stiffness increases throughout the spine, and chest expansion frequently becomes restricted..

There is no satisfactory treatment in modern medicine but acupuncture can slow down process of degeneration and give relieve pain and mobility of joints is maintained , it must be combined with acuyoga exercises and pranayama to give better results
   
OSTEOPROSIS (porous bone)
  Is commonly associated with arthritis, spondylitis and may be cause of underlying pain in arthritis, so it must be treated to relieve pain besides treating arthritis It is due to calcium deficiency especially seen in ladies after childbirth /menopause, adequate intake of calcium as milk or milk product & getting vitamin D from sun bath is essential to avoid it.
   
GOUT
  It is a metabolic disorder of unknown cause leading to increased uric acid in blood as a result of poor digestion and a weakened colon. There may be positive family history . It causes inflammation of the joints and swellings of a recurrent type. Middle-aged men and women post menopause are most affected. There is painful, tender, hot and swallen big toe. The affected foot cannot bear weight and even wrists and knees maybe affected during the acute stage. The attack occurs during mid-night or early hours of the morning. Loss of appetite and slight fever are other symptoms. They may generally lead to formation of kidney stones. Acupuncture along with acuyoga can very effectively cure it .Diet restriction eg tomato ,spinach ,pulses , meat are important .Carrot, cucumber & beet juice is beneficial.
   
HEATH TIPS–BACKACHE AND ARTHRITIS, OSTEOPOROSIS
  • Take calcium in diet–milk and vitamin D( by sunbath ) if osteoporosis.
  • Avoid wrong posture by keeping your spine straight while sitting , standing or lying.
  • Avoid physical and mental stress.
  • Avoid exposure to cold or rainy weather.
  • Use English toilets to avoid straining backache.
  • Regular exercise-acuyoga and Spinal exercises to be done regularly. Donot bend spine
   forward while lifting any object , rather bend knees .
  • Avoid eating gas forming foods like cauliflower and potatoes.
